Before submitting papers to the Mexican diplomatic mission for a visa, you must fill out a visa application form. It’s not at all difficult to do this yourself, and in this publication we will tell you what to enter and where.
The Mexico visa application form is universal; it can be filled out for any type of Mexican visa. The questionnaire may be in Spanish or English. We will fill out the form in English. The questionnaire itself can be downloaded from the link at the bottom of this publication, then printedon one page on both sides(required!), fill out by hand and sign on the second page.
The form should be filled out only in Latin block letters, and preferably with a blue ink pen. Corrections are not allowed. The numbering of blocks below corresponds to the numbering of blocks in the questionnaire itself.
First page
1. Given name(s) — name as in the passport.
2. Last name — surname as in the passport.
3. Second last name — second surname, if it is indicated in your passport.
4. Gender — floor:
- Female— female;
- Male— male.
5. Date of birth — date of birth in DD/MM/YYYY format, specifically 03/18/1985 (March 18, 1985).
6. Age — age, number of completed years.
7. Country of birth — country of birth, indicate as in your passport.
8. Nationality — your citizenship.
9. Number of passport or travel and identity document — series and number of the international passport, as well as:
- a) Country of issue — country that issued the passport; most often, the same as citizenship;
- b) Date of issue — date of issue of the international passport in DD/MM/YYYY format;
- c) Date of expiry — expiration date of the passport in DD/MM/YYYY format.
10. Marital status — marital status:
- single — single/not married;
- married — married;
- common-law — civil marriage.
11. Current address — residential address (street, house, apartment, city, region, country and zip code), transliteration is allowed.
12. Telephone number — contact phone number along with code.
13. Email — email address.
14. Occupation — profession; It is allowed to indicate the name in English or transliterated from Russian. If you are a pensioner, enter Pensioner or Retiree.
15. Company or Institution’s name — the name of the company where you work, or the educational institution if you are studying.
16. Place of residency — state of residence.
17. Legal migration status — whether you have legal status in your country of residence. What this means is that you can apply for a Mexico visa in any country where you have resident status, not just your country of citizenship.

19. If yes, please specify — If there were violations, please indicate which ones.
20. Date of entry to Mexico — Estimated date of entry into Mexico in DD/MM/YYYY format.
21. Point of entry to Mexico (city) — city of entry, specifically Cancun — Cancun, Mexico — Mexico City:
22. Length of stay — Planned length of stay in Mexico:
- Less than 180 days — less than 180 days, in most cases you need to check this box;
- More than 180 days and less than 4 years — from 180 days to 4 years;
- Definitive — exact date.
23. Have you ever been to Mexico? — Have you visited Mexico before:
- Yes— Yes,
- No— No.
24. Have you ever been deported from Mexico? — Have you ever been deported from Mexico:
- Yes— Yes,

25. If yes, please explain — if you were deported, explain why.
26. Purpose of visit — purpose of the trip; In most cases, indicate Tourism.
Second page
Block V on the back of the form — Attached Suppotring Documents. Indicate all the papers that you submit to the diplomatic mission along with the application form. Most often this is a passport, a certificate from employment and a bank account statement — These are the documents listed in our sample form. You can also indicate the papers by transliteration from Russian, for example, Vypiska iz banka.
Date and Place block. Place— city where the Mexican consulate is located. Date— date of filling out the form in DD/MM/YYYY format.
Name and signature of Applicant — name, surname and signature of the visa applicant.
The next two blocks — name, surname and signature of the mother and father who are signing the application form instead of the minor visa recipient.
You do not need to fill out any other blocks in the Mexico visa application; they will be filled out by the consulate employees themselves.
You must paste a recent photograph onto the application form; it is of non-standard size 32x26 or 39x31 mm. If the photo does not fit into the frame on the application form, it can be cropped.
