Until 2020, everything was quite simple in Mexico: if you work officially and have social insurance from your employer (via IMSS), then your family was also automatically treated in the same system without additional payment. No questions asked. But now— No. Everything has changed.
Now for every relative, even if it is your child or an elderly parent, — Separate insurance is required. And, more often than not, pay for it.
What happened to IMSS anyway?
IMSS was originally created as universal protection in case of illness, accidents, maternity and old age — not only for the workers themselves, but also for their loved ones. The system worked as a collective shield. But in recent years, especially after the reforms of 2019-2020, the logic has changed.
Now medical care — This is not an automatic right for the whole family, but a service that needs to be formalized and proven. Kinship — no longer a basis. You need documents, registration, compliance with conditions, insurance experience, and sometimes additional contributions. This looks like a move from the old principle: «you are insured— This means that the family is also insured.
Who is generally allowed to enroll in your insurance?
Service in IMSS is permitted for citizens and residents of Mexico who have an individual CURP code. Guest states are not served by IMSS. According to the law, those who can be considered as your dependents include: spouse, children under 16 years of age (or up to 25, if studying full-time), and in addition to this, parents, if they are truly your dependent. But there are nuances here too.
Firstly, registration in itself does not give the right to service — You have the right to ask for a separate fee for it. Secondly, the registration process falls entirely on the shoulders of the employee himself. The employer does nothing for relatives.
How does all this work out?
If you want to attach a relative — go to your IMSS branch (medical facility) or register online through the IMSS Digital portal. You will have to collect a whole folder of documentation: from CURP and ID cards to marriage certificates, birth certificates and proof of cohabitation.
Some cases — especially with adopted children or guardian relatives — will require even more paperwork and, often, notarial confirmation. In general, without bureaucracy — nowhere.

Why do you still have the right to refuse?
Even if you did everything correctly, IMSS can refuse help. The reasons vary, but the following are common:
- The employee does not have the required insurance period (usually a minimum of 52 weeks — 1 year).
- The dependent was not registered on time.
- The child is over 25 years old and is not studying full-time.
- A chronic disease has been discovered that began before registration (in this case, IMSS has the right to suspend treatment altogether for a year or more).
In other words, formally there is access, but in fact — not always.
There is an alternative: Modalidad 33
To at least somehow compensate for this situation, IMSS offers separate voluntary insurance — Seguro de Salud para la Familia (aka Modalidad 33). This is a paid program that makes it possible to insure, in particular, retired parents or adult children if they do not go through the main system.
The insurance is contractual, must be paid annually, and the price depends on age. Service — the same as in IMSS, but without pensions, without accident payments and without labor guarantees. Some seniors are given discounts, especially if they were previously insured.
Who's feeling a little better?
There are two groups for whom the system is still trying to save face:
- Children: up to 16 years of age (or up to 25 for full-time education) receive medical care without additional fees. The main thing — register on time.
- Pensioners: If a person receives a pension from IMSS, he and his dependents continue to purchase health services. But even here you need to confirm your status, save your insurance number and register your relatives according to all the rules.
For parents of pensioners, the requirement to prove dependency is sometimes simplified. And if a pensioner draws up Modalidad 33, then this does not interfere with his basic rights — You can insure other family members separately.
What if I lost my job?
If you are temporarily unemployed — the system gives a slight head start. There is a concept of continuidad de derechos — rights continue if no more than 8 weeks have passed since the dismissal, and family members were registered earlier. If more than — you'll have to start all over again.
This is especially unpleasant if someone in the family gets sick during the break. Even if it's not your fault, IMSS can refuse service because «registration has been cancelled».
Reality: More exceptions, less universality
It seems that the system has become less social and more procedural. To obtain support — You should not get sick, but first complete the registration quest.
IMSS is gradually moving away from the idea of equal access. First — formal obstacles, then — refusal on medical grounds. This is no longer universal protection, but rather — list of conditions with pitfalls.
What to do if IMSS is not suitable
There are other options, but each with its own disadvantages:
- INSABI — public clinics for everyone outside the system. They work, but they have fewer opportunities, especially for examinations and medications.
- Private insurance — a wide range of services, but expensive and not always available to older people.
- Subscriptions to pharmacies and clinics — like Farmacias del Ahorro. You can subscribe for a fixed fee and arrange consultations and basic tests.
- Charitable programs and DIF (state structure for family development) — help vulnerable groups, but coverage is limited.
The main thing — don't wait until it gets bad
Today IMSS — This is no longer a guaranteed medical shield for the whole family, but a set of rules that should be known and followed. Who wants to protect loved ones — must take care of registration, papers and fees himself.
Timely materials and actions — That's what works best right now. IMSS remained the core of the system, but became more demanding and selective. Without knowing the rules, you are allowed to stay at a closed door — even if there is a doctor behind her.
